Annual Health Updates are required on an annual basis.  If you haven't had an opportunity to complete this for your student online, please take a moment and complete this important form and return it to the school.

If your child has had immunizations in the past year, would you please send a copy of this record to the school?  This can also be faxed to the school: (316)794-4182.

K-A-R 28-1-20 defines immunizations required for any student who attends a public school. School-Requirement-K-12-PDF (ks.gov)

Family Information
I am married and I have two sons. My husband Erik and I met in high school and became friends. We later started dating in college and got married in 2019. My husband is a vehicle technician and specializes in building police cars and emergency vehicles. Our oldest son Maverick is two years old and loves anything Mickey Mouse and playing outside. Kallen is our youngest and was just recently born in September. We also have two dogs, Nova and Lily.
